Paul Ceska was born the son of a security guard inspector and, after graduating from high school, was drafted into the Austro-Hungarian army. Army after graduating from high school. He then began studying at the Faculty of Medicine at the University of Vienna (Dr. med.) and joined the Rudolfina student fraternity.

After graduating, Paul Ceska worked as a general practitioner. He also became chief physician at the State Opera House and from 1930 to 1937, general practitioner at the Theresianum. In 1933, he founded the "Association of Christian-German Doctors" and was its chairman until 1938. In 1934, he became Chief Physician of the Health Care Institute for Employees and Servants of the City of Vienna and, as a representative of the liberal professions, a member of the Vienna City Council (until 1936). In 1935, he took on the role of Regional Medical Council of Vienna.

In the wake of the Anschluss, Paul Ceska was immediately suspended from his duties in March 1938 and retired on April 30, 1938 with reduced pay. He was finally dismissed in October of the same year. He was then drafted into the German Wehrmacht as a doctor.
